Discover compassionate care at Angel's Grace Hospice

Hospice Care

Upon admission, patients will receive care through one of our four levels of hospice care.

Routine Home Care

 Basic level of care provided in your home, assisted living, or nursing home. 

Inpatient Care

 When medical needs cannot be managed in the home. 

Continuous Care

 Medical management in the home for up to 24 hours a day when medically appropriate. 

Respite Care

 Temporary care provided for up to 5 consecutive days in nursing home,  hospice, inpatient facility, or hospital so that the family member or  friend who is the patient's caregiver can take some time off. 


Hospice Care and Palliative care

Hospice Care Program

Hospice is a comprehensive plan of care and support that focuses on the comfort and quality of life for terminally ill patients to live as comfortably as possible in the final stages of their lives. Angel's Grace provides hospice care with compassion and thoughtfulness using pain relief and symptom management strategies.

Hospice Care Eligibility

 Eligibility is determined through a physician's certification stating  the patient has a terminal diagnosis and a prognosis of approximately  six (6) months or less if the disease were to run its normal course. 

Common conditions include:

  • ALS 
  • Alzheimer'sDisease 
  • Cancer (All conditions,and complications.)
  • COPD 
  • HIV 
  • Heart Disease 
  • Kidney (Renal) Disease 
  • Liver Disease (Complications and symptoms.)
  • Neurological Diseases

Who Pays for Hospice Service?

Hospice services are covered from a variety of sources including:

  • Medicare
  • Medicaid
  • Private Insurance Companies
  • Donations & other Private Payments

Patients and/or their families should verify the scope of their individual insurance coverage.

Palliative Care Program

Palliative Care is an interdisciplinary medical care giving approach aimed at optimizing quality of life and mitigating suffering among people with serious, complex and often terminal illnesses.

Palliative Care Eligibility

 Any patients with a serious, long-lasting disease or life-threatening  illness are eligible for palliative care at any stage of their illness.
